Service Details

Supported Living – Provides a flexible partnership that enables an individual to live in their own home with supports from Curative Behavioral Health (CBH). Training activities, supervision, and assistance are provided in areas such as health and safety, budget management, social skills, attending appointments, cooking, shopping, etc.

WHO QUALIFIES

An individual must qualify and receive North Carolina Innovations Waiver services and/or North Carolina TBI Waiver. No specific diagnosis or age limits are required. This service should be requested from the managed care organization (MCO) and can be added into the individual’s Individual Support Plan (ISP).

WHAT PROFESSIONALS ARE INVOLVED

We will work with you and the entire treatment team to determine the individual’s preferences, goals, dreams and help develop a plan of action.
