Service Details

Community Networking – Provides day activities that support the individual in a meaningful day in an integrated setting with persons who are not disabled.

WHO QUALIFIES

An individual must qualify and receive North Carolina Innovations Waiver services and/or North Carolina TBI Waiver. No specific diagnosis or age limits are required.

This service should be requested from the managed care organization (MCO) and can be added into the individual’s Individual Support Plan (ISP).
